29 CFR  1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:   On or about January 20, 2026, at the main building, employees were permitted to operate forklifts with missing mirrors to transport items, including but not limited to bags of  raw material, and other products, without removing  them from service in the  following instances:  a)  On the right side of cab, Forklift #4.  b)  On the left side of cab, Forklift #6.
