29 CFR  1910.124(h)(3): Appropriate first-aid supplies were not located near the dipping or coating operation.  a.  Between February 1 and February 28, 2026, at the facility, employees engaged in metal plating operations were exposed to highly toxic chemicals, including hydrogen cyanide. During this time, the employer failed to maintain a cyanide antidote kit for immediate first aid treatment in the event of exposure.

29 CFR  1910.134(c)(2)(i): An employer may provide respirators at the request of employees or permit employees to use their own respirators, if the employer determines that such respirator use will not in itself create a hazard. If the employer determines that any voluntary respirator use is permissible, the employer shall provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard")  a. On or about January 22, 2026, in the facility, the employer allowed employees to use half mask tight fitting respirators during daily duties. The employer did not provide the employees using respirators with the information contained in the "Appendix D" required by this standard.
