29 CFR  1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) 1500 Chateau Street, Pittsburgh, PA: On or about January 21, 2026 during carpentry work, the employer did not ensure employees used appropriate fall protection systems/devices where exposed to fall hazards of up to approximately 12 feet.

29 CFR  1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) 1500 Chateau Street, Pittsburgh, PA: On or about January 21, 2026 during carpentry work, the employer did not ensure the ladder used to get up and down from the top of the building was properly set up in that the ladder was only extended approximately 6-12 inches above the landing area, exposing employees to fall hazards of up to approximately 12 feet.
