29 CFR 1904.41(a)(1)(i):  For establishments with 20-249 employees in a designated industry listed in appendix A to subpart E of this part, the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form:  In the facility at 40 Rockdale St. Worcester, MA:  As of March 9, 2026, the employer had not electronically submitted information from their OSHA Form 300A or equivalent for calendar year 2025 by March 2, 2026. The establishment employed 36 employees during calendar year 2025, and is classified in NAICS Code 332111 (Iron and Steel Forging) which is listed in appendix A to subpart E.  Komtek Forge LLC was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA Inspection number 1812498, citation number 2  item number 2 and  was affirmed as a final order on August 26, 2025, with respect to a workplace located at 40 Rockdale St. Worcester, MA.
