And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017:  On or about January 17, 2026, employees were exposed to a fall hazard of approximately 4ft 2in., while accessing the roller conveyor of the wheel blaster, because the employer did not provide fall protection.
