29 CFR  1910.1200(f)(6)(ii):  Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  (a) On or about January 29, 2026, and at times prior, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals exposing employees to chemicals such as but not limited to lacquer thinner, which is highly flammable.   Barry's Cabinet & Millwork, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(f)(6)(ii), which was contained in OSHA inspection number 1584781, citation number 1, item number 5b and was affirmed as a final order on July 29, 2022, with respect to a workplace located at 1952 W. Union Ave, B11, Englewood, CO 80110.

29 CFR  1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  (a) On or about January 29, 2026, and at times prior, the employer did not ensure that employees voluntarily utilizing tight-fitting respirators were medically evaluated  prior to usage.

29 CFR  1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by paragraph (l) through a certification that included the name of the operator, the date of the training, that date of evaluation, and the identity of the person(s) performing the training or evaluation:  (a) On and about January 29, 2026, and at times prior, the employer failed to ensure that each operator had been trained and certified on the safe operation and use limitations of forklifts. This condition exposed employees to struck by hazards.

29 CFR  1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service:  (a) On or about January 29, 2026, and at times prior, the employer did not ensure that electrical equipment was free of hazards in that a cord attached to a foot switch that controlled a scissor lift was frayed in multiple areas. This exposed employees to inadvertent electrical contact.
