Maintenance check includes an internal examination, except extinguishers with stored pressure.  a) On or about January 29.2026, at worksite location 37 NW 166th Street , Miami FL. 33169, the employer did not ensure that the portable fire extinguisher Serial Number H16245877 Permit Number FEP 20-000369 was subjected to an annual maintenance check, the fire extinguisher tag label indicated last annual maintenance check was on March of 2023, thereby exposing employees to smoke inhalation and burn hazards.
