No materials or equipment may be placed, either permanently or temporarily, within the exit route:  a) West Chester Operator LLC DBA West Chester Rehabilitation and Healthcare Center located at 800 West Miner Street, West Chester, PA 19382 - On or about January 29, 2026, the employer did not ensure that all exit routes in the maintenance shop within the establishment were kept free and unobstructed from materials or equipment, thereby exposing employees to entrapment and other fire related hazards.

29 CFR  1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":  a) West Chester Operator LLC DBA West Chester Rehabilitation and Healthcare Center located at 800 West Miner Street, West Chester, PA 19382 - On or about January 29, 2026,  the employer did not ensure that each emergency exit was clearly visible and marked by a sign reading "Exit." The exit doors in the Maintenance Shop within the establishment did not have an "Exit" sign, thereby exposing employees working in the Maintenance Shop to entrapment and other fire related hazards.
