OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees due to multiple rack storage units not securely bolted to the floor.  Warehouse: On or about February 4, 2026, the employer did not ensure that multiple rack storage units loaded with merchandise were securely bolted to the floor, which exposed employees to falling objects and potential rack collapse if struck by a forklift, or the uneven distribution of weight of loaded items.

Warehouse: On or about February 4, 2026, the employer did not ensure that rolling platform ladders were inspected to determine structural damage, which exposed warehouse employees that used them to fall hazards.

Warehouse: The employer did not ensure that three damaged platform ladders were tagged and taken out of service and replaced or repaired. Employees were exposed to fall hazards while using structurally damaged platform ladders.
