29 CFR  1926.1052(c)(1):Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stair rail system along each unprotected side or edge:  On or about October 14, 2025, at 2110 Fairview Road, Cleveland Heights, OH, 44106, the employer failed to ensure employees traversing the main stairwell were protected from a fall of approximately 16 feet in that the guard rail system was removed by the general contractor while employees were working from the stairwell.
