29 CFR  1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:   a) On or about 02/17/2026, in the Production area; employees were exposed to caught-in hazards and the unexpected energization or start-up of machinery when performing servicing and/or maintenance activities on equipment such as but not limited to; the Molder, Rounder, Proofer, Mixer and the employer's lockout/tagout program did not include periodic inspections of procedures.  The periodic inspection must: a) be performed by an authorized employee other than the ones utilizing the energy control procedure being inspected.   b) be conducted to correct any deviations or inadequacies identified. c) where lockout is used for energy control, include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected. d) where tag-out is used for energy control, include a review between the inspector and each authorized and affected employee, of that employee's responsibilities under the energy control procedure being inspected, and the elements set forth in paragraph (c)(7)(ii) of this section.   ABATEMENT DOCUMENTATION REQUIRED.
