The employer did not ensure that each operator had successfully completed training consisting of a combination of formal instruction, practical learning and evaluation required by the standard, prior to operating the Jungheinrich EJE 120, Caterpillar DP40K, Yale NR035EBNS36TE095 and orange Toyota powered industrial trucks.

The data plate, that identifies the various rated capacities of the forklift at different load configurations, was missing.
