29 CFR  1910.101(a): The employer did not determine that compressed gas cylinders under his control were in a safe condition to the extent that can be determined by visual or other inspections conducted as prescribed in the Hazardous Materials Regulations  On or about January 6, 2026, the employer did not ensure that a pressurized fire bottle was maintained in a safe condition. The pressurized fire bottle was outside of the required hydrostatic testing interval.  Employees were exposed to struck-by and impact hazards due to the sudden release of pressurized components.

29 CFR  1910.303(b)(7)(i):Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment.  On or about January 6, 2026, the employer did not ensure that unused openings in electrical panel were effectively closed. Electrical panel box had a missing knockout on the bottom right side.  Employees were exposed to electrical hazards.
