29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.) At the worksite, located at 1747 Grand Isle Dr Brandon, Fl 33511; On or about 2/11/26, the employer exposed employees to struck by hazards, in that, employees were walking around the construction site with no head protection as roofers worked overhead throwing debris and working with shovels and scrapers.

29 CFR  1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):  a.)  At the worksite, located at 1747 Grand Isle Dr Brandon, Fl 33511; On or about 2/11/26, the employer exposed employees to struck by hazards in that unauthorized and untrained personnel were authorized to operate a GEHL RS6-34 Telehandler Forklift that was utilized to lift trash can bins and materials approximately 18ft and 24ft in order to perform roofing operations on the 2nd level of a residential apartment building.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.) At the worksite, located at 1747 Grand Isle Dr Brandon, Fl 33511; On or about 2/11/26, the employer exposed employees to fall hazards of approximately 18ft and 24ft , in that, fall protection was not provided for employees conducting roofing operations on a residential building.  Colony Roofers, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1808936, citation number 1, item number 1 and was affirmed as a final order on 7/7/2025, with respect to a workplace located at located at 13723 Atlantic Blvd, Jacksonville, FL 32225.
