29 CFR 1910.23(b)(9): The employer did not ensure that ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury. On or about February 11, 2026, employees engaged in commercial mechanic activities, located at 714 N Washington Street, Papillion, NE 68046, were exposed to fall hazards of up to 3-feet above the lower level in that the employees were using a damaged ladder that had not been inspected before the initial use of the ladder.

29 CFR 1910.23(b)(10): 29 CFR 1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects as immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with § 1910.145 and removed from service until repaired in accordance with § 1910.22(d), or replaced. On or about February 11, 2026, employees engaged in commercial mechanic activities, located at 714 N Washington Street, Papillion, NE 68046, were exposed to fall hazards of up to 3-feet above the lower level in that the employees were using a damaged ladder that had not been inspected before the initial use of the ladder.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about February 11, 2026, employees engaged in commercial mechanic activities, located at 714 N Washington Street, Papillion, NE 68046, were exposed chemical hazards to include but not limited to: Acute hazards: skin/eye irritation and Chronic hazards: cancer, in that the employer has employees working with used motor oil from motor vehicles and parts without having provided the employee with training on the health hazards associated with the hazardous chemical use, how to read and understand the safety data sheets or the first aid measures necessary upon exposure.
