29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a.) Joey Merchant DBA Joey Merchant, Econo Lodge, 2404 E State St. Hermitage 16148, PA, on or about 12/09/25 and times prior thereto: Employees were repairing a portion of the low slope roof at the Econo Lodge without any protection from fall hazards. Employees were observed working within a foot of the edge of the roof and exposed to fall hazards of approximately 20 feet to ground level.    ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR  1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)   a.) Joey Merchant DBA Joey Merchant, Econo Lodge, 2404 E State St. Hermitage 16148, PA, on or about 12/09/25 and times prior thereto: employer did not provide training to employees that were performing roof work operations on a commercial flat roof. Employees were exposed to fall hazards of 20-feet above ground level. Employees did not have fall protection training.   ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. 29 CFR  1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:   a.) Joey Merchant DBA Joey Merchant, Econo Lodge, 2404 E State St. Hermitage 16148, PA, on or about 12/09/25 and times prior thereto: Employees used the handrail on the second tier staircase as a means of access and egress from the roof of the Econo Lodge. Employees were exposed to approximately 10 foot falls to ground level.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
