29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about February 17, 2026, the employer did not ensure that effective machine guarding was in place to protect employees from the hazardous areas of the H2 Horizontal punch press (HOVIS). Guarding was not in place to protect employees from the moving machine gears, rotating cam shafts and levers, rollers, and ram area. Employees are tasked with the operation of the equipment which includes making adjustments and changes during operation. The hinged barrier guard contained an interlocked safety switch which prohibited operation while opening. This interlock was bypassed and did not function. b) On or about February 17, 2026, the employer did not ensure that effective machine guarding was in place to protect employees from the hazardous areas of the H1 Horizontal punch press. Guarding was not in place to protect employees from the moving machine gears, rotating cam shafts and levers, rollers, and ram area. Employees are tasked with the operation of the equipment which includes making adjustments and changes during operation. A hinged barrier guard is in place; however, the guard is not interlocked, nor does it completely cover the hazardous area.
