An employee used a Cotterman 6-Step Rolling Ladder (Model H-1083-10) with visibly deteriorated rubber stabilizing feet that were not identified prior to use. As a result, employees were exposed to injury hazards associated with the use of a ladder with visible defects.

On or about February 9, 2026, in Hangar 3, the employer did not ensure that a mobile ladder stand remained stationary while occupied. An employee performing aircraft maintenance from a Cotterman 6-Step Rolling Ladder (Model H-1083-10) was injured when the ladder platform moved while occupied during the task. As a result, employees were exposed to injury hazards associated with ladder displacement while working from mobile ladder stands.
