29 CFR 1910.28(b)(1)(i )(c):  The employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by personal fall protection systems:  a. On or about February 18, 2026, at the marina: employees were exposed to a 19' fall hazard while fixing a flag that was wrapped around a flag pole, and the appropriate personal fall  protection was not used.

On or about February 18, 2026, at the marina:  the employer did not provide fall protection training for employees exposed to 19' fall hazards, when fixing a flag that was wrapped around a flag pole while standing on forklift forks.
