Taft Road, Liverpool, NY 13088: An employee utilized the scissor lift guardrails  which were less than 18 inches wide as a work platform.

29 CFR  1926.451(g)(1)(vii):For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) On or about 2/20/2026 at the site located at 4979 W Taft Road, Liverpool, NY 13088:  An employee utilized the scissor lift guardrails as a working platform without the use of fall protection. The employee was exposed to a fall of 17 feet.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On or about 2/20/2026 at the site located at 4979 W. Taft Road, Liverpool, NY 13088: An employee running new data cable for a new installation was kneeling on a cable tray attached to the ceiling without the use of fall protection. The employee was exposed to fall of 17 feet.
