29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a. On or about February 23, 2026, at the jobsite: employees were exposed to 19 feet, 8-inch fall hazards, in that fall protection was not used while installing roofing tiles on a  7:12 pitch roof.

29 CFR  1926.1053(b)(22):Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  a. On or about February 23, 2026, at the jobsite: the employer exposed employees to fall hazards from ladders from 8-feet to the ground, in that hoist were not provided to employees and had to carried buckets of cement mortar that could cause a balance disruption and fall when carrying them.
