An employee fell approximately 20 feet from the top level of a two-story residential house with a pitch of approximately 9-in-12, resulting in a fatal incident.  The employer failed to ensure employees were utilizing personal fall arrest systems and/or devices. This condition exposed employees to fall hazards of approximately 20 feet.    b) On or about September 3, 2025, the exposing employer failed to ensure employees in the process of conducting roofing activities on a residential roof located at 7524 E. 121st Place, Thornton, CO, were protected from falls. An employee fell approximately 20 feet from the top level of a two-story residential house with a pitch of approximately 9-in-12, resulting in a fatal incident.  The employer failed to ensure employees were utilizing personal fall arrest systems and/or devices. This condition exposed employees to fall hazards of approximately 20 feet.
