29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: a) On or about September 2, 2025, the exposing employer failed to ensure employees engaged in roofing activities on a residential roof located at 7524 E. 121st Place, Thornton, CO, were trained in the appropriate use of fall protection. Equipment includes, but is not limited to, personal fall arrest systems or devices. This condition exposed employees to fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about September 2, 2025, the exposing employer failed to ensure employees in the process of conducting roofing activities on a residential roof located at 7524 E. 121st Place, Thornton, CO, were protected from falls. An employee fell approximately 20 feet from the top level of a two-story residential house with a pitch of approximately 9-in-12, resulting in a fatal incident. The employer failed to ensure employees were utilizing personal fall arrest systems and/or devices. This condition exposed employees to fall hazards of approximately 20 feet. b) On or about September 3, 2025, the exposing employer failed to ensure employees in the process of conducting roofing activities on a residential roof located at 7524 E. 121st Place, Thornton, CO, were protected from falls. An employee fell approximately 20 feet from the top level of a two-story residential house with a pitch of approximately 9-in-12, resulting in a fatal incident. The employer failed to ensure employees were utilizing personal fall arrest systems and/or devices. This condition exposed employees to fall hazards of approximately 20 feet.
