29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a) New residential construction building located at 361 Golucke Way, Lot 32, McDonough, GA/Roof -  On or about February 25, 2026, the employer exposed employees to struck-by hazards in that, the employer failed to ensure employees were provided with eye protection while operating pneumatic nail guns and engaged in roofing activities.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New residential construction building located at 361 Golucke Way, Lot 32, McDonough, GA/ Roof - On or about February 25, 2026, the employer exposed employees to a fall hazard of more than 18-feet to the lower ground level, in that the employer failed to ensure fall protection systems were provided to employees installing shingles on the roof.

29 CFR 1926.552(a)(4): Hoisting ropes were not installed in accordance with the wire rope manufacturers' recommendations.   a) New residential construction building located at 361 Golucke Way, Lot 32, McDonough, GA/Roof - On or about February 25, 2026,  the employer exposed employees to struck-by hazards, in that the material hoist was used with a nylon, climbing type rope and not the wire rope per manufacturer's recommendations.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) New residential construction building located at 361 Golucke Way, Lot 32, McDonough, GA/ Roof : On or about February 25, 2026, the employer exposed employees to fall hazards of approximately 18-feet to the lower ground level, in that employer failed to ensure the ladder used to access the roof of the structure was extended at least three feet above the upper landing surface.
