29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: American Seafoods F/T Northern Eagle, Maintenance Shop: On or about February 25, 2026, employees performing maintenance activities utilizing a Hydraulic Press were exposed to injury due to lack of guarding. OSHA Instruction: Shipyard Employment "Tool Bag" Directive - CPL 02-00-162 allows for the applicability of 29 CFR 1910.212(a)(3)(ii) in the ship building and ship repair industry.

American Seafoods F/T Northern Eagle, Portside Main Deck AFT: On or about February 25, 2026, employee(s) used a fixed ladder with less than six inches of toe clearance to traverse across the main deck. The ladder was considered unsafe because there was not adequate clearance behind the rungs. This condition potentially exposed employee(s) to slip, trip and fall hazard(s).
