29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about January 29, 2026, the employer did not establish an energy control program consisting of energy control procedures, employee training, and periodic inspections. Employees were not protected from the unexpected energization, startup, or release of stored energy while performing servicing and maintenance activities, such as, but not limited to setting up, adjusting, inspecting, cleaning or unjamming of machines or equipment, making adjustments or tool changes. Equipment serviced and maintained in the facility includes but is not limited to the Ameri-Seiki TC-2L (C6) CNC lathe. Hazardous energies present on the equipment include but is not limited to electrical and pneumatic.

On or about January 30, 2026, following a work-related incident that occurred on or about January 29, 2026, which resulted in an amputation, the employer failed to report the incident to OSHA within the required 24 hours of being notified. The employer reported the incident to OSHA on or about February 10, 2026.

Employees are handling oils, lubricants and coolants while working with various CNC machines. These oils, lubricants and coolants on consist of but are not limited to Mobil DTE 24 Ultra, Mobilith SCH 220, TRI-Kote, TRIM E923 and Slide/Way.
