29 CFR  1910.219(c)(4)(i): Projecting shaft ends did not present a smooth edge and end, and was not guarded by nonrotating caps or safety sleeves:  a) 1251 S. Front St, Camden, NJ - Picking Station M34 Belt - The M34 belt tail pulley shaft was modified with a metal flange welded to the projecting shaft end, therefore the shaft end was not smooth and was unguarded, thus exposing employees to rotating parts.  An employee working for the company suffered severe injuries when his clothing came in contact with the rotating unguarded projecting shaft end and rotated the worker several times. This condition was observed on or about February 27, 2023.  NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
