29 CFR 1926.21(b)(2):  The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to their work environment to control or eliminate any hazard(s) or other exposure to illness or injury:   a) Work zone ? Intersection at N. Perry & W. Pike St.: On February 26, 2026, the employer exposed employees to struck-by hazards while they were adjusting a valve within an intersection that remained open to vehicular traffic. The employer failed to ensure that employees were instructed in the recognition and avoidance of hazardous conditions associated with performing work in an active roadway, including the proper use, placement, and importance of traffic control devices to protect themselves from moving vehicles.
