Effective January 1, 1998, body belts are not acceptable as part of a personal fall arrest system: Roof - On or about February 28, 2026, an employee was exposed to fall hazards exceeding six feet, while wearing a climbing/body belt, instead of a body harness.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds (22.2 kN) per employee: Roof - On or about February 28, 2026, an employee fell to his death. The anchorage system was not secured with the correct amount of nails or screws, the lifeline was not attached to the anchor's D-ring, and the anchor was incorrectly installed, in that it was positioned perpendicular to the peak of the roof.
