Employees were offered respirators without being provided Appendix D information, without ensuring respirators were cleaned or stored properly, without confirming that the respirators were appropriate for the conditions of use and without medical evaluation prior to use.

Employees were assigned work involving presumed lead?containing surfaces without a Hazard Communication program, without effective training on chemical hazards, and without procedures for understanding exposure risks, despite being offered PPE.

Employees disturbed painted surfaces without having been trained on lead hazards, exposure pathways, protective measures, or safe work practices, despite the foreseeable presence of lead?containing materials and the employer's provision of respirators and PPE.
