29 CFR 1926.451(c)(3): Supported scaffold poles, legs, posts, frames, and uprights shall be plumb and braced to prevent swaying and displacement. A) 826 Central Avenue, Yonkers - NY: Employees were exposed to fall hazards of approximately 13-1/2-feet while working on a pump jack scaffold that was not braced at the bottom. The employees were not wearing any type of complete fall protection devices either. The employees were exposed to falls, scaffold collapse hazards, on or about, March 2nd, 2026.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access: A) 826 Central Avenue, Yonkers - NY: Employees were exposed to fall hazards of approximately 13- 1/2 feet while using an extension ladder to access a ladder pump jack scaffold. The extension ladder that was used, was only extended approximately 2-feet above the landing and not the recommended 3-feet as required by the manufacturers instructions, on or about, March 2nd, 2026.
