29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Location:  Production Area  Instance A:  Lock Out / Tag Out procedures were not developed and utilized for servicing of the box making machine such as un-jamming the machine.  Instance B: Annual inspections / audits of LOTO procedures were not performed on employees servicing and maintaining the box making machine.  Instance C:    The company did not have LOTO training records for all operators of the box making machine.

Location:  Box making machine in production room  The employer did not ensure that an ingoing nip point created by a chain and sprocket at the bottom of the machine was guarded.
