29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)  Maintenance Office:  On March 4, 2026 and times prior to, employees operate a Delta ShopMaster (serial number 065849W3083 Model No. DP200) drill press for installation of brackets for chairs.  The employer did not provide guarding for the rotating spindle/chuck of the Delta ShopMaster drill press.   No abatement certification or documentation required.

29 CFR  1904.29(b)(7)(v): Needlestick injuries and cuts from sharp objects that are contaminated with another person's blood or other potentially infectious material were not recorded on the OSHA 300 Log as a privacy case.  (a)  200 Hospital Drive, Spencer, WV 25276:  On or about March 4, 2026, the employer did not omit the employee's name from the OSHA 300 Log and instead recorded the employee's name.  No abatement certification or documentation required.

29 CFR  1904.41(a)(1)(ii): The employer's establishment had 250 or more employees during the previous calendar year but the employer failed to electronically submit information from the establishment's OSHA Form 300A by March 2.  The establishment had an average of 350 employees during the 2025 calendar year.  (a) 200 Hospital Drive, Spencer, WV 25276:  On or about March 4, 2026, the employer had failed to submit their Calendar Year 2025 OSHA Form 300A information to the Injury Tracking Application (ITA) prior to March 2, 2026 deadline.    No abatement certification or documentation required.
