Observed on or about 03/04/26.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Observed on or about 03/04/26.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) Corners of Argyle & Shefa Chaim Avenues, Lakewood NJ:  A company worksite worker was exposed to fall hazards, in excess of 15 feet 1 inch when working/walking from a supported scaffold's planked side brackets that were missing end stops. 29 CFR  1926.451(g)(3)(i): Vertical lifelines were not fastened to a fixed safe point of anchorage independent of the scaffold:   a) Corners of Argyle & Shefa Chaim Avenues, Lakewood NJ: The anchor point used for an employee's Personnal Fall Arrest system was a structural member within an end frame of a prefabricated, tubular welded framed scaffold's third buck exposing this employee to falls (in excess of 15 feet 1 inch to the ground surface below) and crushing hazards. Observed on or about 03/04/26.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
