29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  a) On or about February 11, 2026, the employer did not provide training to each powered industrial truck operator to ensure that they are competent to operate the company's Toyota Forklift Truck (Model: 50-8FGU30), exposing workers to crushing and struck-by hazards.  b) On or about February 11, 2026, the employer did not provide training to each powered industrial truck operator to ensure that they are competent to operate the company's Combilift multi-directional pedestrian reach stacker (Model: COMBI-WR4), exposing workers to crushing and struck-by hazards.
