29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about March 3, 2026, the employer did not ensure that effective machine guarding was in place on the Acme Gridley multi spindle screw machines. Sliding barrier guard doors allowed for employees to access the hazardous areas of moving machine components while in operation. Interlock mechanisms were either not present on the equipment, bypassed, or in need of repair.    b) On or about March 3, 2006, the employer did not ensure that effective machine guarding was in place on the Cosen C-300NC chip conveyor/ auger area. Employees are exposed to the moving machine components during operation. A barrier guard was not in place to prevent employees from reaching into the area.   c) On or aabout March 3, 2026, the employer did not ensure that effective machine guarding was in place on the Cosen C-300NC band saw. Employees were not protected from the hazard of the moving unused portion of the blade.   d) On or about March 3, 2026,  the employer did not ensure effective machine guarding was in place on the manual lathe located in the screw machine area of the facility. The lathe did not contain a chuck/ chip guard to prevent employees from hazards during operation.   e) On or about March 3, 2026, the employer did not ensure that effective machine guarding was in place on the tube/ pipe cutoff saw. Barrier guarding was not in place to prevent employees from the moving machine components while in operation including but not limited to the blade area itself.   f) On or about March 3, 2026, the employer did not ensure that effective machine guarding was in place on the backside of CNC machines. Barrier guarding had been removed and not put back into place prior to operation on both CNC numbers 461 and 466.
