29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:     On or about March 6, 2026, while installing framing on a residential new build located at Lot 12, W. Sunrise View Dr., Akron Ohio (between built home #453 and #448); the employer allowed employees to work from elevated heights while standing on a pallet of work materials attached to the fork of a JCB (green) #507 telehandler (sunbelt rental). No fall protection was worn at this time.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about March 6, 2026, while installing framing and felting of a residential new build located at Lot 12, W. Sunrise View Dr., Akron Ohio (between built home #453 and #448); the employer failed to ensure that employees working from elevated heights were protected from a fall from height hazard of up to 30 feet in that employees were not wearing a personal fall protection harness nor were there any other fall protections in place.
