29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Warehouse area labeled Piler 1 - On or about February 26, 2026, the employer exposed employees to amputation and caught-in hazards in that, the employer failed to ensure that point of operation for the BC28-4 /Piler 1 machine was guarded while employees were removing waste strips for the machine.
