29 CFR  1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  Employees engaged in replacement of propane cylinders on powered industrial trucks where not utilizing appropriate eye protection at the facility located at 7400 N Broadway, St Louis, MO, on or about March 10, 2026 and at times prior thereto. Powered industrial vehicle operators were changing propane fuel cylinders on power industrial trucks with vented goggles and no face shield. The workers were exposed to potential skin and eye injuries (frostbite) that could have resulted from contact with liquid propane.

29 CFR  1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.  Employees engaged in replacement of propane cylinders on powered industrial trucks where not utilizing hand protection at the facility located at 7400 N Broadway, St Louis, MO, 63147, on March 10, 2026 and at times prior thereto. Powered industrial vehicle operators were changing propane fuel cylinders on power industrial trucks without hand protection. Workers were exposed to potential skin burns (frostbite) that could have resulted from contact with liquid propane.
