29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:   On or about March 9, 2026, at a work location located at 13365 Ridge RD, N. Royalton Ohio 44133; the employer did not ensure eye protection was worn by employees using pneumatic nail guns and hand tools to install roofing shingles, exposing the employees to struck-by hazards to the eyes.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about March 9, 2026, at a work location located at 13365 Ridge RD, N. Royalton Ohio 44133; the employer did not ensure employees engaged in roofing activities over 6 feet were protected from falls. Employees were exposed to falls of 12 feet above the ground while tearing off and installing roofing shingles on a roof without utilizing any form of fall protection.
