29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.  a) On December 11, 2025, an employee fell resulting in injuries that required inpatient hospitalization. The employer did not report the hospitalization of employee to OSHA within 24 hours.  The injury was reported to OSHA on March 11, 2026.  Note: Because the abatement of this item is documented in the case, file the employer is not required to submit certification or documentation of abatement of this item.
