29 CFR  1926.760(a)(1): Employees engaged in a steel erection activity who are on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level were not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:  (a)Sentry, LLC at 501 Pheasant Ridge Drive, Watertown, SD 57201: On or about and at times prior to March 11, 2026, the employer did not ensure that employees were protected from falls while installing roof insulation on a low-slope commercial property.  This condition exposed employees to fall hazards greater than 15 feet.

29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a)Sentry, LLC at 501 Pheasant Ridge Drive, Watertown, SD 57201: On or about and at times prior to March 11, 2026, the employer did not ensure that employees were protected from falls while standing and climbing on the mid rail of a scaffold platform to gain access to the steel structure's roof located on the east side of a commercial building. The employees were climbing on a scissor lifts railing which was raised more than 15 feet high by a Skyjack Scissor Lift, Model Number: SJ6832 RT 4 X4, Serial number: A200010863 to gain access to the roof. This condition exposed the employees to fall hazards greater than 10 feet.
