29 CFR  1910.119(f)(1)(i)(G): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and did not address the startup following a turnaround, or after an emergency shutdown:   On or about December 6, 2025, the employer did not develop and implement written operating procedures that provide clear instructions for closing and applying caps to two air utility openings after conducting a cleaning/clearing (thawing) operation on Valve XY 287-V.

29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about December 6, 2025, the employer did not develop, document, and utilize written procedures to control the release of hydrogen from the L2 Unit during cleaning/clearing operations on Valve XV 287-V.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about December 6, 2025, employees were exposed to fire and explosion hazards associated with the release of hydrogen during servicing of Valve XV 287-V  to thaw the valve/line.  The employer failed to implement energy control application steps as the unit was not isolated [per the 11910.147(d)(3) requirements].  As a result, the remaining applicable energy control elements involving the application of LOTO devices [(d)(4)], stored energy was not restrained and/or otherwise rendered safe[(d)(5)(i)], and the verification of isolation [(d)(6)], were not implemented to protect employees from hazards caused by the release of hydrogen from the unit.
