29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about December 6, 2025, employees were exposed to fire and explosion hazards associated with the release of hydrogen during servicing of Valve XV 287-V  to thaw the valve/line.  The employer failed to implement energy control application steps as the unit was not isolated [per the 11910.147(d)(3) requirements].  As a result, the remaining applicable energy control elements involving the application of LOTO devices [(d)(4)], stored energy was not restrained and/or otherwise rendered safe[(d)(5)(i)], and the verification of isolation [(d)(6)], were not implemented to protect employees from hazards caused by the release of hydrogen from the unit.
