29 CFR 1910.178(m)(1):  Trucks shall not be driven up to anyone standing in front of a bench or other fixed object.   a. Dock 36 - On or about March 10, 2026, the employer did not ensure that powered industrial trucks were operated in a manner that protected employees from injury. A powered industrial truck (Raymond Model 415?C35TT, Serial No. 415?23?0081815) was used to drag a pallet along the ground while on the forks and was driven up to an employee standing inside a fixed trailer, resulting in the pallet striking and crushing the employee's right foot between the pallet and stacked boxes along the trailer wall.  b. Dock 37 - On or about March 19, 2026, the employer did not ensure that powered industrial trucks were operated in a manner that protected employees from injury. A powered industrial truck (Raymond Model 415-C35TT, Serial No. 415-23-0081790) was used to drag a pallet along the ground while on the forks and was driven up to two employees standing inside a fixed trailer.
