29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Roof of new residential construction - Lot 3, 108 Weldon Rd. Stockbridge, GA: On or about March 17, 2026, the employer exposed employees to a fall hazard of approximately 20 feet above the ground level, in that the employer failed to ensure that fall protection systems were provided to employees installing shingles on the roof.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Roof of new residential construction - Lot 3, 108 Weldon Rd. Stockbridge, GA: On or about March 17, 2026, the employer exposed employees to fall hazards of approximately 10 feet to the lower ground level, in that employer failed to ensure the ladder used to access the roof of the structure was extended at least three feet above the upper landing surface.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  a) Roof of new residential construction - Lot 3, 108 Weldon Rd. Stockbridge, GA: On or about March 17, 2026, the employer exposed employees to fall hazards at approximately 20 feet to the lower ground level, in that the ladder used to access the roof of the structure was missing its halyard (rope) and was not removed from service.
