Swann Ave., Tampa, FL - on or about 03/16/2026, an employee removing tree branches from a roof  was exposed to an approximate 20 foot fall hazard in that fall protection equipment was not installed or utilized.

Swann Ave., Tampa, FL - on or about 03/16/2026, employees working from an Platform Basket aerial lift, model # PB22.10, serial # PB9881 were exposed to an approximate 20 foot fall hazard in that, the employees were not provided with a personal fall arrest and/or restraint device system.
