29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On or about March 19, 2026, at the worksite 506 Wolf Street, Syracuse, NY 13208: The employer failed to ensure an employee working on a roof with an unprotected edge, 11 feet above the next lower level, was protected from falling by use of personal fall arrest system, safety net system, or guardrail system.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about March 19, 2026, at the worksite 506 Wolf Street, Syracuse, NY 13208: The employer failed to ensure an employee using a portable extension ladder to access a roof had the side rails of the ladder extended 3 feet above the access point.
