29 CFR  1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check:  (a) By the  back entry, on or about February 13, 2026, and at times prior thereto, workers were exposed to safety hazards in that a portable fire extinguisher had not had annual maintenance since August 2024.  (b) By the front entry, on or about February 13, 2026, and at times prior thereto, workers were exposed to safety hazards in that a portable fire extinguisher, #MR- 620070, had not had annual maintenance since August 2024.

29 CFR  1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly:  (a) By the  back entry, on or about February 13, 2026, and at times prior thereto, workers were exposed to safety hazards in that a portable fire extinguisher had not had any monthly inspections during 2025 or 2026.  (b) By the  front entry, on or about February 13, 2026, and at times prior thereto, workers were exposed to safety hazards in that a portable fire extinguisher, #MR- 620070, had not had any monthly inspections during 2025 or 2026.  The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1663131, citation number 2, item number 1, and was affirmed as a final order on 10-19-2023 with respect to a workplace located at 3031 Tongass Avenue, Ketchikan, AK  99901.
