29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) At the excavation located at 1761 Atlanta Hwy, Cumming, GA 30040, on or about 03/24/2026, employees were observed working inside an excavation measuring approximately 4 feet 11 inches in depth, 14 feet 6 inches in width, and 50 feet in length. A ramp had been placed within the excavation; however, it was installed at a slope that did not provide safe access, as it was too steep to serve as a safe means of egress.
