29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) New commercial construction located at 2905 Castleberry Road, Cumming, GA 30040: On or about 03/25/2026, employee engaged in metal framing activities using a JLG aerial lift was exposed to ejection/fall hazards of approximately 23 feet to the lower level, in that the required fall protection/fall arrest system was not used while positioned inside the aerial lift basket.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.  a) New commercial construction located at 2905 Castleberry Road, Cumming, GA 30040: On or about 03/25/2026, employees performing  metal framing activities from an aerial lift had not been trained to recognize fall hazards while engaged in framing work using a JLG Aerial Lift at heights exceeding 23 feet above the lower level.
