1926.20(b)(2)  The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  Location 359 Plymouth Street Halifax MA. 02338   On or about March 24, 2026 employees were exposed to fall hazards where guardrail systems, safety net systems or personal fall arrest systems were not used to protect from falling off the side of the building.  The employer did not have a program to conduct and document frequent and regular inspections of their worksites to ensure that workers were protected from falls.

29 CFR  1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761:  Location: 359 Plymouth St. Halifax MA. 02338  On or about 24 March, 2026, employees were exposed to falls while erecting a systems-engineered metal building where the training provided did not address all aspects of the recognition and avoidance of fall hazards.
